WESLEY - NURSE PROVIDES
* Personal Health Counselor - Discuss individual health concerns and expore options to meet needs. * Health Educator and Advocate - Provide Information and instruction on a topic, skill, health question, health management, and resources. Acts as a motivator, champion, and coach. * Facilitator - Provides access to health supplies, equipment, transportation, financial assistance, pharmaceuticals assistance, and food/clothing/benefits/resources. Organizes groups within the congregation. * Referral Service - Makes appropriate referrals. Serves as liason to community resources and services. * Spiritual Nurture - Prays with and for persons. Encourages faith and instills hope. Communicates love and respect, and acceptance. |
